Sacramento County Project Launched to Improve Transition of Youth from Foster Care to Adulthood

A one-year demonstration project has launched in Sacramento County, California, to support foster youth in more successfully transitioning out of care by providing them direct, on-line access to their own records and documents. The project, a joint effort between The Children’s Partnership, AltruIT 2.0, Inc., and Aspiranet, will expand HealthShack, an existing, youth-designed electronic health and personal record system, to foster youth between the ages of 18 and 20 in Sacramento County. The project is,being funded by The California Wellness Foundation.
